All purchases of Upgrade Health Inc. dba Live Forever Health’s® treatments are final because our products are medications that, once packaged for you, cannot be used for another patient due to our pharmacy’s regulations. Prescription medications cannot be returned for reimbursement, and thus we are unable to accept returns for refunds, even if they are unused.
Term and Termination Clause for our Monthly Packages
1. Initial Term:
This Agreement shall commence on the Effective Date and shall remain in full force and effect for a minimum period of three (3) months (“Initial Term”). During the Initial Term, neither party may terminate this Agreement for any reason.
2. Continuation and Renewal:
Following the completion of the Initial Term, this Agreement shall automatically continue on a month-to-month basis, unless terminated in accordance with the provisions herein.
3. Termination After Initial Term:
After the Initial Term has been completed, either party may terminate this Agreement by providing ninety (90) days’ written notice of termination. The termination shall become effective at the conclusion of the ninety (90) day notice period.
4. Effect of Termination:
During the notice period, both parties shall continue to perform their respective obligations under this Agreement, including payment for all services rendered. All outstanding fees and balances shall become immediately due upon the effective date of termination.
However, we want you to receive high-quality products that are ready to use. Therefore, we will replace items if they are defective or damaged. If you need to exchange a product, please submit a ticket via online form, and we will coordinate with you to exchange your order.
For auto-renewing products and services, your payment method will be automatically charged at regular intervals as described during the checkout process until you cancel your subscription. You may cancel your auto-refills at any time up to forty-eight (48) hours before the applicable renewal processing date of your auto-refill through the Service logging into your account and selecting the cancel option.
